would it be wise to have the wisdom tooth removed?

: It is questionable if that wisdom tooth is giving you so many issues. Certainly your clogged eye duct is unrelated. Sinusitis may be caused by so many different factors and can be related to your headaches. Have you consulted with an oral surgeon and an ENT?  Condition of your teeth? Are you grinding or clenching? Explore these possibilities.

Deviated septum: A deviated septum is not an uncommon problem. It can lead to nasal obstruction, snoring and even sinus issues. You only need surgery if you are having symptoms. Be sure to have your nose evaluated by a board certified otolaryngologist.

Good question: Rhino-sinusitis is a viral/bacterial or rarely a fungal infection of the paranasal sinuses caused by obstruction of the very narrow entrances to the sinus cavities from the nose.  As such, a deviated septum would have to be so severe as to obstruct those openings.  However, your surgeon may opt to correct such a deviated septum, if access to the sinuses is obstructed by the septum.

Irrigate/followup: Deviated septum and sinus surgery have come a long way over the past decade. Now most patients don't need packing and don't get bruised or swollen on the outside. But it important to follow instructions and not strain or do strenuous activities for a few weeks after surgery. Irrigate with saline regularly and keep your followup appointments so your surgeon can make sure you heal properly.

Both: Septal deviation may be present without the presence of cysts or polyps. Sometimes if there are intranasal masses, they can cause pressure on the septum and cause deviation. Alternatively, if septal deflections are such that they obstruct the passage of mucous from the sinuses, this can lead to sinusitis and ultimately the development of cysts and polyps.

i have a 3 cm tumor in my sinus. they think it's due because of my deviated septum. what are the chances it could be cancerous?

Unlikely: Unless you know the features of this tumor in sinus(?Polyp) is hard to be sure. Your ENT doctor can advise you further. If there is any anxiety about its nature, a repeat x-ray of the sinus in 3 months can be more assuring that it is likely to be benign. Cancer of the sinuses is an uncommon cancer but it does occur. Usually it will produce blood stained nasal discharge and some local(facial pain).

is it safe to fly on a plane with a deviated septum? i suffer with sinus barotrauma.

Yes: The only issue on plane flight is pressure equalization when there is a blocked passageway.  Deviated septae cause breathing obstruction but not blockage so flying is perfectly safe with this condition.

Time Heals Most: It is not unusual for your symptoms of nasal obstruction and inability to smell to get worse before they get better after surgery. This is often due to intranasal swelling & improves with time. That being said, some people have impaired smell before surgery and don't realize it.  On the other hand, damage to your smell nerves during surgery & persistent sinusitis are known risks. See your surgeon.
