i have degenerative disc disease dislocation of 3 lumbar discs and disc herniation at l5/s1 and disc bulges on l3/4 l4/5 my doctor says he can't op why?

Unclear : You may possess abnormal films, yet not have clinical signs of nerve disfnctn., and may do better with conservative patience.  You could possess a risk comorbidity, and the benefit might be not great enough to risk surgery.  Have you had prior surgery, as subsequent operations could create arachnoiditic scarring, which could be far worse.  Maybe get additional opinions.

l3-l4 5mm disk bulge, l4-l5 9mm bulge, dehydrated disks, moderate central canal stenosis. what are my options for treatment? symptoms are lower back pain at rest, waking up middle of night, sometimes radiating pain. will decompression machine help?

: You should try non-narcotic pain reliever and physical therapy first.
Please follow with a spine orthopedist.

Based on symptoms.: Slight disc bulge is probably present in most every 53 y/o spine.  A very non specific finding. Treatment would be based on symptoms.  If there are no symptoms directly referrable to your l4/5 issue then simple exercise / perhaps pt and some NSAID / tylenol (acetaminophen) / mechanical precautions is all you need.

may the protruding part of the disk l4-l5 (4.2mm) be sliced off using some minimally invasive surgical procedure? i have dynamic stenosis at l4-l5. strong pain in lower right buttock after sitting for 2+ hours. conservative treatment did not help.

It depends: upon how miuch of the "stenosis" is actually due to the herniated material from the disc and how much from BONY stenosis (in the canal itself or the foramenal (side openings between the verterbrae) where the nerve exits. The kind of "focal" MICROSCOPIC surgery you describe is the LEAST disruptive of your spine architecture and offers your BEST CHANCE!

rest
physical therapy
medications such as lyrica
translaminal/transforaminal epidural injection
surgery.

6mm herniated disc,l4-l5.  lower back pain and now into hip.  is steroid normal initial treatment. what's best, very painful at times.

Therapy Usually fist: if the herniation is posterolateral, then epidural or foraminal steroid injection(s) is a possible answer.  But your symptoms suggest that the herniation is more central.  In case of lateral herniation, the pain would go down to the top of your thigh and your knee( L4 nerve root caught at L4-5).  So I think you should try low back strengthening, traction, or a McKenzie certified therapist first!

my dr
is treating me. with pain meds i also was on many. on many other meds but no luck i have very active life and i cant do my daily task anymore bc pain

Back pain: Please continue to see your doctors but add a mind-body treatment into the mix.  Search John Sarno, MD on the web to learn ways to use your psyche to help.  It's not "all in your mind" but pain runs through the brain.  Peace and good health.

after a back/si injury x-ray & mri shows thoracic c. fractures, bulge@l-5-s1, tear@l4-l5 & levoscoliosis at l3-l4. after moving new dr say x-ray is normal!? still uneven w/pain & muscle weekness spasms. how do i seek treatment or get diagnosis?!?

A spine orthopedist is recommended and you should obtain all the radiology results on CD when seeing the orthopedist.

No: An intervertebral disc is like a jelly donut.  It is a tough outer shell with a jelly-like material inside.  A bulging disc is when part of the disc pushes out into the space where the spinal cord runs or where the nerves leave the spinal cord.  The jelly stays inside the donut.  In a herniation the jelly is squeezed out of the donut.  The jelly-like material is irritating and can cause pain.

m 33 yrs old male: changes of osteoarthritis in ls spine. pid seen at l4-l5 and l5-s1. diffuse fatty liver seen is it permanent? pilates is cure?

Lumbar disc : Pid or prolapsed intervertebral disc is a herniated disc.  Discs can be retained within the capsule or free fragments.  Discs not causing debilitating leg or back pain can definitely be helped by physical therapy or pilates.  Sometimes selective nerve root blocks can reduce pain so patients can undergo physical therapy.  Surgery is the last resort.  See your primary care for fatty liver.
