Based on symptoms.: Slight disc bulge is probably present in most every 53 y/o spine. A very non specific finding. Treatment would be based on symptoms. If there are no symptoms directly referrable to your l4/5 issue then simple exercise / perhaps pt and some NSAID / tylenol (acetaminophen) / mechanical precautions is all you need.

Integrative care: A herniated disc can cause back pain as well as foot pain by physically pressing on the nerves that travel to the foot and by irritating them through the extrusion of pro-inflammatory disc materials. Physical therapy and other conservative measures, including medication management, are considered first line treatment. If these fail, your physician may recommend epidural steroid injections.

Disc Bulge: If you have a disc bulge, but do not have any symptoms, then no treatment is necessary. Exercise, diet control, and strengthening are part of a healthy life style and lower back.
