we are trying to figure out why i am randomly feeling sharp pain months after a deep cut on my left gluteus muscle. the pain has been off and on for months as i've healed, but it's extremely strong right now. why?

Nerve damage?: It could be that you damaged a nerve? If it is sharp and comes with certain movements, this may be the situation. A nerve conduction study would help define this.

Anti inflammatory: I send my patients with this type of problem to the chiropractor. Using Ibuprofen is also helpful. Can take 4-6 weeks of treatment to get better

Can't tell: You need a diagnosis. Rheumatologist might be right (but what is prp?). If there is question about diagnosis, you may need any or all of the following:
1. Hip x-ray
2. Pelvic MRI (musculoskeletal)
3. Emg
4. Blood tests for connective tissue diseases. Acupuncturists, quite often quite capable of relieving pain, are not noted to be diagnosticians. Even if treatment relieves pain, it doesn't diagnose.

when i sit  i have a sharp pain in my right buttock, doesnt radiate down my leg. had an mri that showed tears in the gluteus medius but ortho said it doesnt mean anything. had si and piriformis injection, no improvement. any ideas for dx tests?

Diagnosis: Will not be needed, as you have one that is consistent with imaging and your symptoms. The confusion comes from communication with ortho that should read as not surgically correctable in his or her opinion. This type of lesion will at least partially heal, it will take long and physical therapy may help.

.: Manual massage of the muscles will help. Medication to help with pain will also decrease the spasm too.

Pain meds and see MD: You can take some non steroidal pain medicine if you are not allergic to it like Aleve (naproxen) or ibuprofen . If not relieve of pain and recurrences of pain consult your doctor get examined and may need X-rays or other imaging if indicated to make a diagnosis and appropriate treatment

Tendonitis and......: enthesopathy-fancy term for tendonitis.  Specifically inflammation of the tendon at the bone attachment or the origin of the muscle in question. Origin is where muscle originates from or anchors to, so when it contacts it can move the joint (in this case the hip). Periostitis is inflammation of the periosteum, the outer fibrous covering of bone.  Contains many nerves and vessels. Painful condition.
