is it "better" to have sclerotic bone lesions, than lytic ones with bone metases? is there still a greater chance of fracture of the affected bone, whether it is lytic or now sclerotic?

Good question: Excellent question.Unfortunately not an easy answer. But overall, either will do structural damage to the bone and leave it more vulnerable to a fracture or collapse.The lytic ones are quite a bit weaker,however, and can break with little or even no trauma.Both can be very painful and difficult to treat.Also,remember the source: breast,prostate,lung,thyroid, and kidney  are more apt to go to bone.

: This MRI result is not for you, I assume.  The finding is quite abnormal but medical history is necessary for a meaningful advice. Next step will likely be bone vs bone marrow biopsy.

Yes: This is not uncommon.  If you have no history of cancer, they are probably benign.  If you are worried, a bone scan could be done.  If negative, i wouldn't worry about them.

Coincidental finding: Bone island,  enostosis, is  benign bone tumor mostly encountered as an incidental and asymptomatic finding. They are round and small (2 to 20 mm) intramedullary condensations composed of dense lamellar cortical bone.

: Sclerosis is a noun and sclerotic is an adjective. Yes, you can say bony sclerosis or sclerotic change of the bone. Cortical aspect of the bone should be smooth and defined so if it is not, the image will be described as seen which could be irregular, coarse, bumpy, etc...

No: These do not regress as long as the cancer is there, unless the cancer is overgrown by a tumor with different biology. The cancer creates and maintains the abnormal remodeling that results in the blastic lesion.

Prob Not: Bone islands are common.  They are usually seen coincidentally on plain xray.  They have a typical benign look.  If you're concerned get another xray in 3-6 months.
