Rarely: A C-section scar can sometimes be seen on transvaginal ultrasound. Scar tissue inside the endometrial cavity cannot be seen on conventional ultrasound. This kind of scar tissue can interfere with pregnancy. Scar tissue usually cannot be seen in the bladder.

Yes: As long as you have had an ultrasound, the likelihood of a more serious condition causing the pain is unlikely. Scar tissue typically forms actively for 3-6 weeks after injury subsiding thereafter. If the gets worse or does not subside after 6 weeks i would recommend a repeat ultrasound to check on status if scar tissue or other conditions causing the pain.

D&C: With hysteroscopy would be the next step, chances are the opening of the uterus (cervix) is stenosed(blocked) at its inner opening- happens often after ablations. Tissue/blood fill up the cavity and aren't passing well through the blockage. Hysteroscopy D&C can open this up and get a good look to rule out other issues, once in awhile can be something worse.

See details: It sounds like you are describing Asherman syndrome.  This is scarring of the endometrium.  This frequently follows a d&amp;c that was performed for bleeding or infection. Progestin will not work.  The scars need to be lysed (broken up so to speak) through a hysteroscope.  Afterwards, a device is placed in the uterus to prevent new scars and you are treated with estrogen to rebuild the endometrium.

Endometriosis : Endometriosis is stimulated to grow by estrogen.  Ovaries make estrogen.  If you still have cells that are endometriosis, they could be responding to estrogen from your ovaries.  The best way to find out if your pain is from endometriosis is to take a medicine called Lupron (leuprolide) for three months.  Lupron (leuprolide) makes estrogen low.  If you feel the pain less, it was from endometriosis.  If the pain does not get better it was not from endometriosis.  Your ob/gyn can help you with this.

Probably: If you have scar tissue inside your uterus (asherman's syndrome), then removal of the scarring will certainly improve your fertility.

Adhesions: Any time you have abdominal surgery you risk forming adhesions, which are really just a kind of abdominal scar tissue.  If you form adhesions that obstruct your bowel, etc. You may need another surgery to take down the adhesions.  It is a common complication, but having to have another operation is not.
