when i lay in my back on a the floor to do pt, it feels like i am trying to grind my sacrum bone into the floor. foam mat no help.very painful! why?

there may be a : Variety of reasons. So make sure you are doing your exercises with proper techniques. Make sure you follow specific instructions provided to you by your therapist. If one specific program leads to increased pain, avoid that particular exercise, as there may be alternative exercise that can allow you to achieve same goal.

my lowest sacrum bone sticks out and i can feel it it is like a ball, it that bad, and should i talk to a doctor? i can not do correct stomach crunches or roll on my back, i have to lean sideways in order to not feel discomfort.

The : The bottom of the sacrum is called the tail bone or coccyx.  Sometimes people are born with them sticking out or sometimes they stick out after an injury (fracture).  In severe cases the coccyx can be removed.  I would discuss this with your primary care doctor.  Good luck!

i can physically feel my sacrum/ coccyx bones when i touch the bottom. is it normal to feel them? it sometimes hurts when i sit down. what about that?

A donut and patience: Coccydynia or tailbone pain is most likely and has many causes from unknown (most common) to direct trauma, infection, and tumor.  It can be treated with a pain reliever or anti-inflammatory medication such as ibuprofen.
Limiting sitting for prolonged periods is suggested along with use of a padded cushion or "donut" to protect the tailbone.
It can take weeks to months to resolve so be patient.

catscan for microhematuria revealed a focus of sclerosis in left superior sacrum representing a bone island or chondroid lesion and 2nd smaller sclerotic focus in mid left iliac bone presumably of similar etiology. is this of any concern?

Ortho oncology: Multiple lesions are concerning. You should see an Orthopaedic oncologist for the proper work up which may involve a biopsy. I would not take any chances.

mri show hairline facture on each side of sacrum 1 on pubic bone from 7/5 fall. still pain limited movement, how much longer to recovery?

needs reevaluation: The fracture should have healed by this time and the pain should have resolved. It needs an orthopedic reevaluation. And possibly a new MRI or ct scan.

2 cysts at s2 (racepinephrine) 2cm each cause scallop of bone at sacrum n lamina. hip stiff n muscle spasm. feel  lump at tailbone when sit. should go for surgery?

Don't is safe way: Tarlov cysts in this region are not uncommon and are usually not a cause for pain unless one is dealing with soft or osteoporotic bone which can occasionally be subjected to pressure changes by a large cyst creating a fracture of the bone which then becomes painful.  See an orthopedist to evaluate you symptoms for the source.

how common is bone erosion? what could possibilities be for change over a months time on xrays? was the tip of coccyx but now halfway up sacrum eroded

Context matters: Whoever ordered the X-rays should be answering your questions. It does not matter what it could be or how likely it is. What matters is what it is and how to treat.  If your doctors don't know yet, they certainly will be working to find out the cause. If they haven't told you anything yet, it could be they don't know yet. If they do know, they will tell the patient or his/her designee.
