Rule out active TB: Calcification in the apical lungs in a TB endemic region is old healed TB until proven otherwise . However active TB needs to be ruled out if you have any symptoms of shortness of breath, unplanned weight loss, bloody cough, chronic cough etc. It can be done with sputum studies or a ct scan.  This " feature" cannot be removed by surgery, drugs, diet. .It is akin to a well healed injury.

Emphysema: is a disease of the lung which is either caused by smoking or a genetic disorder. Emphysema destroys lung tissue and creates holes called blebs that look like Swiss cheese holes. By themselves, they are not dangerous, but they can lead to pneumothorax (collapsed lung). Since they are associated with smoking, they can be the location of future lung cancer. So please STOP SMOKING.

How do you feel?: Whether you are fit to work or not depends on what you do for work, and whether or not they have policy or regulations regarding employee health requirements. X-ray findings alone are generally not an issue; that said, if you are short of breath and cannot perform as needed, then fitness may become an issue. Pulmonary function testing may be required. Advise immediate follow-up CT/MRI + contrast.

It means there: are small abnormal cystic air-filled spaces in the upper right lung.  It is likely an incidental finding that has been there for years, and is of no clinical significance.  However, it is possible that it is an early manifestation of a more progressive process, so speak to your doctor about the significance in your particular situation.

Need more informatio: Need more information, More medical and social history. smoking or not. Any other medical disease at present.
These findings are on MRI of shoulder and not a dedicated MRI of chest
 So You need to be seen by a MD or Pulmonary to go over your history, physical exam and more work up if  needed

june 2023- chest xray- results- biapical pleural-parenchymal scarring and question 1 right lung apex nodule. chest xray was due to having covid in 11/2022. radiologist suggests repeat pa and do apical lordotic views. can covid cause these findings?

: I doubt those findings are due to covid-19 infection if there is no abnormal finding in the lower lung fields. Repeat CXR with PA and apical lordotic views may give more information. If you are at risk for tuberculosis exposure in the past, then a tuberculin test should be done. CT scan of the chest may be necessary. Good luck.

why does tuberculosus lodge at the apex of the lungs?

More oxygen at apex: There is more ventilation and less blood flow at the apex of the lung.  Since tuberculosis is a strict aerobe it grows better in the better oxygenated upper lobe where less of the oxygen is carried away be the blood.  The lower lobes of the lung have more blood flow and less ventilation so there is less oxygen to feed the TB because the blood carries more of the oxygen into the body.
