It sounds like the technician is seeing blood in the cavity of the uterus. The sonogram also should be interpreted knowing what problem you are struggling with. If the problem is heavy periods then schedule a visit to discuss options

should this be concerning? i am 48 years old

No: I would label this as normal uterine and endometrium findings.  Not sure why the ultrasound was done.
