Rashes are hard to: diagnose without a visual exam or more history.  If your child has a fever or seems ill, he should be seen urgently by your doctor.  If the skin is itchy, this is unlikely to be  infectious and more likely to have an allergic basis.  Many viral rashes do not itch.  Fifth disease, a virus, causes the cheeks to be very flushed and the skin on extremities to be blotchy &amp; child acts well.

Likely dermatitis: You probably have dermatitis.  This is inflammation of the skin with itching or soreness, of various causes.  The causes range from allergic to autoimmune.  If the symptoms persists and cause significant pain or discomfort, you need to see your PCP for  Dermatology referral.

Cold Urticaria: Cold induced urticaria occurs whenever the skin is chilled, by cold air or cold water, or by direct contact with cold items like ice or a cold drink can.  It is not a sign of immunocompromise.

Herxheimer reaction?: Depends in part on reason for penicillin. The most common need for PCN by injection is syphilis. If so, you may be having a Jarisch-Herxheimer reaction (named for the docs who first described it). Billions of dying syphilis bacteria release toxins. It means the PCN is working and you’ll be cured. It will clear up in 1-2 days. Ibuprofen can help.  Contact doc or clinic if uncertain.

Nerve problem: This is most likely due to a nerve irritation under the skin.  There are multiple causes for this and seeing your podiatrist would help by getting you on the right medication to stop the itching and burning.
