Thrush: A white rash in the back of the mouth may be caused by a yeast infection, "thrush."  It is treated with a special medicine.  Other possibilities include infections and inflammation.  If condition lasts more than a few days, see a doctor.

likely :canker sore: If you also feel some pain and discomfort in the lesion/pimple like area, and it stays less than, or around  14 days, then most likely it. Is canker sore, or aphtus,  there are simple ways to remedy that.

Try : Try to get a mirror and look at it, did you get a cut there?  If it is a cut it may cause that feeling of a loose piece of skin.  As it is painful it is important to be sure that it isn't an infection.  If it continues with swelling or pain then you need to have a medical evaluation.  If it is growing, becomes red, or you get a fever then you must be seen as an emergency.

The tooth may be infected with cavity bacteria, or rotting of dead nerve if the tooth has been dead. It may suggest the root has fractured.
