hello. i would like to ask about the causes and treatment for per rectal bleeding in a 23 years old male. it has been 6 months, on and off, bright red, slight pain in anus, symptom relieved by increased fluid intake and fibre, no loss of weight or appetit

Hemorrhoids: Nice description of hemorrhoid bleeding.  Keep a good balance of fiber (5-6 servings of fruit / vege a day) and hydration (enough liquid to have clear urine 4-5 times a day - not what you get with beer or coffee) and he should have durable relief.  Otc hemorrhoid rx may help.  If not, or if the bleeding or pain worsens he may have a higher grade of hemorrhoid or fissure and shoul see a doc.

eat more fibers and drink a lot ow water anf juices and gaterade like fluid eat more fruits and vegetables and have a bowel movement daily
if persiste check with your doctor.

May need imaging: To exclude possibility of persistent abscess. Notify your surgeon. Some pain is normal but if not i proving may reflect need for additional treatment.

Many options: This is a limited format to discuss all the options for the treatment of rectal pain.  I would recommend evaluation with a practitioner that specializes in pelvic pain.   www.pelvicpain.org 
I would also discuss treatment options further with you doctor.  The fibroids may or may not be causing the rectal pain.  In addition keep stool soft and avoid constipation.

Not necessarily: Acetomenophen, whether taken orally or per recum, is absorbed into the bloodstream and circulates to its site of action. There is no advantage to take the drug rectally if you have rectal pain because the drug does not produce its effects at the sie of administration.

Rectal pain: It sounds like it may be a rectal fissure which is a tear in the rectal mucous membrane. It should heal up i'm a few more days bit if it doesn't you should have it evaluated.

After that h&p, further evaluation with appropriate lab tests (including urinalysis) and perhaps a consultation with a general surgeon or gastroenterologist may be required.  To make a final diagnosis, lower endoscopy or anoscopy may be helpful.

Need evaluation: If you have persistent or recurrent blood with bowel movements this should be evaluated. Most of the time espiecially in a young person this is due to a benign cause such as hemorrhoids or an anal fissure, and the cause can frequently be found and treated with a simple office visit with anoscopy...See your doc or a colorectal surgeon.

Many things: Many things can cause the symptoms you are describing.  If you are constipated it is a good idea to increase the amount of fluids in your diet.    If you have heartburn an over the counter medication such as Tums (calcium carbonate) may provide relief.  Otherwise, see your doctor for a proper exam.
