: This is a very concerning constellation of cardiac arrhythmias &amp;warrant an evaluation by a cardiologist with a specialty of electrophysiology. Extensive cardiac testing will be needed as well as appropriate treatment and follow-up. Patients with both bradycardia and tachycardias occasionally require a permanent pacemaker in order to tolerate some of the medications needed to prevent tachycardias

Pvc : Your symptoms can be associated with several conditions at the same time. Here a list: thyroid disease, fibromyalgia, dehydration, side effects of supplements, drug use, anxiety or a psychosomatic condition. Seems like your heart is not the problem. I would speak to a  good internist who will spend some time talking with you, looking at the results. Explore physical and emotional conditions.

Pause: If you really had a pulseless /20sec., that would be very abnormal. You should be checked and perhaps have a holter monitor to see if that really is happening.

when i stand up , i get light headed all the time , i'm on 25 mg if antenolol for heart pvcs , pacs . what is causing this and is it serious issue?

When I stand up: It may be due to hypotension, you may have to adjust the dosage of  atenolol. Have your blood pressure checked while lying down sitting and standing. Good luck.

Palpitations: can be an ominous sign of heart ischemia (low oxygen/blood supply to muscle) or arrhythmia; or due to more benign causes such as stress. Neurologic and cardiac symptoms in the context of new infection (potential cause of new fever, although yours is low) are concerning for early systemic shock, which occurs in major or blood-born infections.
