Palpitations: can be an ominous sign of heart ischemia (low oxygen/blood supply to muscle) or arrhythmia; or due to more benign causes such as stress. Neurologic and cardiac symptoms in the context of new infection (potential cause of new fever, although yours is low) are concerning for early systemic shock, which occurs in major or blood-born infections.
