Similar procedures: By definition, a pulpotomy is removal of the pulp within the crown of a tooth, leaving the pulp in the roots behind.  This is done commonly in primary teeth because the roots are thin and risky to clean out.  It can preserve a baby tooth for a few years.  Pulpectomy is done as an emergency procedure in permanent teeth to relieve pain and is the first step in root canal therapy.

Sometimes: When a  pulpectomy is done, the dentist is doing the best he or she can do to help relieve your pain.   Don't think of the pain as an on our of switch, but rather a sliding scale.   The procedure should have started a process of improvement, so look for the trend of continuing improvement.   If your pain is nonresponsive or escalating, this is not normal.  If concerned call for an evaluation.

Not a good idea: A pulpectomy is used to access an infected pulp tissue through the center of the tooth, even if a crown covers that tooth. Ideally, you want to preserve the most tooth structure, circumferentially, as possible.  Drilling the side of a tooth would only serve to undermine good struture and lead to the failure of the crown and tooth.  Remember, completing  the root canal filling is vital, as well.

Not uncommon: Pulpectomy is just the first stage of treatment. Completion of Root Canal Therapy will resolve the problem. Speak to you own dentist to see when he\she wants to see you next and if anything can be done for relief of pain meanwhile.

Pulpectomy: Usually it is but each case is different. You may have an infection or there may still be tissue in the canals. Therefore call the dentist who treated you for advice. You may need to be seen sooner than your scheduled appointment. Your own dentist can prescribe antibiotics if necessary and an appropriate pain medication meanwhile.

good question!
local anesthesia works by blocking chemicals that move into and out of nerves in the in contact with the local anesthetic. This means that in order for it to be effective and last long enough for the procedure in question it the anesthetic must be the right  drug, location and conditions for them to work. Now, assuming your dentist injected the right drug in the right location, the only thing remaining is the conditions present in the area. For reasons outside the scope of your question, the environment around an infected area is difficult to anesthetize. In general it is an acidic environment. Local anesthetics work poorly or not at all in acidic tissues. So, this is why you felt what you felt. One last thing, local anesthetics are not good at blocking pressure. So even during a perfect block, you may feel something. Just not pain. Hope that helps. Thanks for the question.
