Treat cause.: Best option is to treat underlying cause.   If cause is chronic pulmonary embolus, treatment is anticoagulation.  If cause is chronic hypoxia  treatment is oxygen,  and maybe cpap. If the cause is a  heart valve or structural heart disease then repair is the best bet.  For those without a clear cause diuretics, oxygen, exercise and Coumadin (warfarin) are often recommended.

Not a fixed percent: You can't say that the systemic arterial pressure is always 5 times the pulmonary arterial pressure because each can vary.  But the branching of the pulmonary arteries tends to lower resistance of blood flow and keep the pulmonary arterial pressures lower.

Different arteries: Mean arterial pressure is calculated by adding the systolic pressure to twice the diastolic pressure and dividing by 3. Systemic pressure is your normal BP (actually aortic pressure).  Example 120/70. Map = 87. Pulmonary artery pressure is the pressure in the pulmonary artery (carries blood from the right ventricle to the lungs). Normal is 35/15. Mean pulm artery pressure = 22.

Why do you ask?: Movement and position affects  many of the body'[s actions.  Blood pressure, circulation, respiration, pulmonary pressure, digestion.  How much depends on each person and their state of health &amp; what meds they may be taking, etc., If you have lung disease, it will affect pulmonary pressure in any position and in varying degrees depending on severity. What are you worried about specifically?

PA pressure: The body position can affect the PA pressure by altering venous return to the right heart. If the venous return is decreased in the individual patient by the reclining position relative to supine then the PA pressure may be somewhat lower than in the supine position. The actual measurement technique is the same.

Echo: There are many technical aspects to echo measurements. Pulmonary artery pressure can vary also depending on hemodynamics and physiologic state. In general however , if measured at rest the PAP wouldn't be 25mmHg different. From out here can't judge further not knowing the technician(s) or the lab.

Pulmonary HTN: Elevated blood pressure in the right heart and lungs is common and results from many causes, including problems with the function of the left heart and problems with the lung's own blood vessels.  Echocardiography can usually but not always make the diagnosis. An excellent review of this topic is in J Am Soc Echo 2013 by Bossone et al.

Interesting question: Elevation of pulmonary pressures can result from volume overload, particularly when there is left heart failure. A normal right heart, however, can compensate for increased volume for a period, preventing increases in pressure.  When persistent stresses lead to structural changes in the cells of the right heart, it cannot compensate further and increased pressure results and by echo indices of PHT
