Otoplasty is best: One device is an ear band with a molded earpiece, but this is really on for babies, when their ear cartilage is still moldable.  The device has to be worn all day. An otoplasty can be useful to correct issues like a prominent ear where the entire ear is pushed forward or a cup ear deformity where there is a lack of one of the normal ear folds.  Consult a facial plastic surgeon for a consultation.

Earfold = an implant: Earfold is a curved piece of metal that is inserted under the skin and in front of the ear cartilage. It is meant to form an antihelical fold when it is absent to correct certain types of prominent ears. The gold standard is surgical correction, which can be done under local or anesthesia. Both can work: I typically use a bandage type head drawing immediately after surgery. This helps place and secure the ears and absorb drainage right after surgery. However, once this is removed, i don't expect patients to redress their ears in the same way. Using a headband (and their are many different styles/approaches) allows the patient to take this on and off, adjust as directed by your surgeon. Ask your dr.

The : The length of the otoplasty surgery depends on a lot of factors, though a rough estimate is anywhere from 2 to 6 hours.  It depends on the anatomical reason of why your daughter's ears are prominent and the various techniques that can be used to fix each problem.  You should get an opinion from plastic surgeon or pediatric otolaryngologist in your area who does this kind of operation frequently.

Prominent : Prominent ears are a common problem faced by facial plastic surgeons.  There are most commonly two problems that occur to cause a prominent ear.  First, there is too much of the auricular bowl (the large bowl shaped area behind the ear canal).  Second, the antihelical fold is insufficiently folded.  This fold is the ridge of cartllage that looks "y shaped and runs vertically in the normal ear.  The surgical procedure to repair this problem is called otoplasty.  During otoplasty an incision is made behind the ear and usually some small part of the ear cartilage is removed or sculpted in some way.  Also, most surgeons use some permanent sutures to help reshape the ears.  So, to answer your questions: yes they can be made smaller with a minor day surgery and usually some tissue is removed.
