Heart: I am not sure why you would think this would be a heart issue. The right upper back would be a very peculiar referral pattern for cardiac disease. Moreover it would be quite unusual for someone your age to have heart disease but if you have trouble with exercise or ever pass out with activity your heart should be checked promptly.  Right upper back pain more likely related to your lungs or muscles

Need to be seen by : your physician right away. There's not enough information from your history to determine the actual answer. Need more information on your recent past history, medical conditions, medications, and any other incident leading up to this point. Several options: It may respond to over the counter pain relievers (follow the package instructions and make sure you do not have a condition or take other things that might make them unsafe). Heat and cold may help. You can find back exercises on line, but you may benefit from a few sessions with a physical therapist, who can show you good technique in a home program. Regular aerobic exercise is very good too.

Given the large number of possible causes, you should see your GP for evaluation.

No.: You probably have just musculoskeletal  problems rather than fibromyalgia.Fibromyalgia has widespread musculoskeletal pain accompanied with fatigue, sleeping problems as well as anxiety and mood issues. You need to consult with your doctor for further evaluation.
