ALTE: Alte (apparent life threatening event) is an acute, unexpected change in an infant's breathing behavior associated with 
apnea( no respiratory effort ), 
color change ( cyanotic or pallid), 
marked change in muscle tone & 
choking. Only half of the cases, cause (s) is found, e.g., gastroesophageal reflux, seizure.  There is no causal relationship between alte and sudden infant death syndrome.

Airway blocked: You get a large chunk of food or other object stuck in the trachea (windpipe)  and no air can get into the lungs causing suffocation and ddeath.

Accepted Answer

Make sure!: First make sure that the person is actually choking.  Ask them directly, they won't be able to talk but can confirm it using signs or nodding.  If the person is choking but it is coughing  they should be left alone as it means they are still being able to get air in and are trying to clear the obstruction.  In the case when the person cannot breath the heimlich maneuver can be used.

Not often: Only when there is complete obstruction of the airway is choking fatal.  If you are talking about the kind of choking used in sexual arousal; this can be very dangerous and should be avoided at all times.

GERD/Fistula: If this has been on going since birth there is the small possibility that an undiagnosed tracheo-esophageal fistula may be present. 
Gastroesophageal reflux maight also cause this to occur.

Unusual: A very enlarged thyroid (goiter) can cause choking symptoms.  Sometimes smaller growths can cause difficulty swallowing and occassionally a feeling like something is getting caught.  Most big goiters show an obvious swelling in the neck, but sometimes a goiter will grow backwards or toward the chest and may not look so obvious. Have a doctor check it out.

No: There are several possibilities including reflux, asthma and sleep apnea. It should be pretty easy to figure out the cause

Gasping: The most likely cause of waking up choking and gasping for air is obstructive sleep apnea. See a doctor for examination and possible sleep study!
