May not b approp: Depending on the cause and degree of thrombocytopenia a platelet transfusion might not be apprpriate and for some diseases like thrombocytopenic purpura giving platlets is like pouring fuel on a fire and should be avoided.

Variable, worse &gt;65Y: The prognosis in AML is variable/ is based on age(worse in &gt;60 yr olds) and cytogenetic changes in the leukemic cells.
Low platelets are common and often fluctuate as observed in this case.
Find out the genetic subtype of his AML. Achievement of complete remission(CR) with the first cycle of chemo(4 weeks time) is a good sign. What date did he start his chemo?

Yes?: Not sure if you are asking or telling us that he is being flown back to the uk.  Platelets that low will cause abnormal bleeding and platelets are necessary. There are several conditions that cause low platelets and a hematologist would be the specialist to see. The initial interventions could be done in egypt and once more stable could be flown home.

Low platelets: Low platelets may be caused by many things, but "itp" or idiopathic thrombocytopenia is a common autoimmmune cause, in which platelet transfusions are of limited value as the platelets get "chewed up" steroids may help but may need a bone marrow test first. So if DX is in question then return to uk for more accurate DX may help, but only if he is "stable' for travel.

This : This sounds like thrombocytopenic purpura.  It can be caused by a variety of conditions, but sometimes there is no cause identified (in which case it's called idiopathic.) if she is not being seen by a hematologist, she definitely should be. Has she been exposed to heparin?  This is a common intravenous or subcutaneous medication that can cause a similar picture in patients who are sensitive to it.

Transfusion: Plasma transfusions are given when a patient's blood is "too thin" and doesn't clot well. Platelet transfusions are given when a patient's platelet counts are too low, a condition which also prevents effective blood clotting.

Intravenous water : Soluble vitamins can do it. Linus pauling worked with a scottish physician and found that large doses of vitamin c helped cancer patients. I/v vit c gets higher concentrations. Trouble is that vitamin therapy has not yet become an accepted approach.
