S/he will need to know about other medical illnesses you've had, other medications you've been on, personal history, use of substances, family history, etc.  There may be tests of thinking. All this is to understand what's causing you distress, in order to treat the problem.

Ask your doc: Concierge &amp; direct care medicine have a different connotation &amp; interpretation by each physician. What's included in price for one practice may not be included in another. Some still charge your insurance while others don't. If you're interested in or been offered a concierge practice, best to ask that doc directly exactly what s/he includes in his/her fees. And get it in writing.

Mental Health pro: Would be helpful to you.   That might be a clinical psychologist for therapy or a psychiatrist for medication if appropriate.  Sounds like anger management is a central issue and there are strategies for working with that which are helpful.  Deal w/in now before it gets worse. Peace and good health.

Focused evaluation: I'm not sure what kind of evaluations you've had about this problem -- and it's not clear what contributes to "i can't [get involved]."  a 15-minute medication management-oriented appointment isn't likely to allow opportunity to explore whatever is limiting you. You might ask for in-depth psychological evaluation and testing.  This could help flesh things out so effective help can be given.
