Complete vs. split?: Peroneus longus tendon injuries are more common in 40+ yr olds. The tears can be complete or longitudinal splits down near your ankle. If rest, therapy and nonoperative treatment fail to improve symptoms of pain, surgery may be indicated to debride the injured tissue or repair the tear/avulsion or longitudinal split. An orthopaedic foot and ankle specialist can guide your decision.

Depends: Many peroneal tears can be treated with rest, bracing or other forms of temporary immobilization plus a rehabilitation program. Surgery can be utilized if you fail conservative care. The lack of visualization of the anterior talofibular ligament means that the MRI slices were not thin enough to capture it or it has likely been injured in the past. Many function well with an abnormal atfl on mri.

I would try: And figure  out what stresses are being placed on the tendon.....For whatever the reason it seems the peroneals have alot of pressure on them.....I would think of how to destrees  or remove load from them or whatever is causing them to fail.

i got an mri of my foot done and it reported that i have moderate plantar fascitis with partial tear of proximal central cord. i also have mild peroneus longus tendinosis. the doctor has recommended prp. do you agree?

PRP: Platelet rch plasma injections are a reasonable option for your problem. However no guarantees can be given for this type of treatment and unfortunately insurance does not cover the cost in most cases. Other conservative options include cortizone (hydrocortisone) injections, stretching, orthotics,  and other modalities including night splints. Surgery is rarely necessary.

Rest: Tendonitis is treated by relative rest (cease from all activities that give pain), physical therapy, and anti-inflammatories (ibuprofen, alleve).  Sometimes immobilization is neede in a cast or boot.  A cortisone shot if often helpful, but cannot be done in some areas of tendonitis.

Experience counts.: It goes without saying that every patient's condition should be evaluated before applying general rules, but, if the laceration is distal (toward the toes)  to the knot of Henry, then surgical repair is probably best. Find an experienced surgeon.

Pain: Pain with active and passive range of motion to your great toe.  There may also be pain to the back of your ankle or pain with plantarflexing of your ankle while moving your great toe up and down.  Hope that helps.
