Yes: So he is saying that when someone has HTN at a young age. It puts their children  at higher same risk. Your children should be made aware of the risk, and they should eat healthy, exercise regularly and maintain a normal body weight....And no smoking. That will help reduce risk.

ASA &amp; MI prevention: Different groups have different thoughts re use of aspirin for prevention of first heart attack(MI)in people w/o cardiovascular disease(or equivalent).No doubt aspirin can prevent MI, but at increased risk of major bleeding events.  USPSTF advises no aspirin for men younger than 45. ASA advised when MI risk for men 45-79 or stroke risk for women55-79 &gt;GI bleed risk(3%-12% depending on age/gender.

What's up?: Something is agitating you. Is it asthma? Are you in pain? (i see you take hydrocodone and amitriptyline) - pain will make your heart beat faster. Are you anxious? Do you wheeze? Do you smoke?  I see you had a heart cath - was it normal? If so, nitro will not likely be of any value to you.  If you're feeling stressed (we often ruminate lying in bed), it can make you short of breath with fast hr.

: You should ask your cardiologist if the echocardiogram changes would account for your symptoms. If the changes don’t account for your symptoms, you should see a pulmonologist for an evaluation to see if your symptoms might be pulmonary (lung) related. You may need more tests like pulmonary function test, CT scan of your chest, etc. Good luck!

: It’s important to keep your blood pressure controlled. You should see a lung doctor about the crackles and chest tightness. If this isn’t heart failure then you need to know what is causing this as it may be causing your dyspnea or at least a contributing factor. So, see a lung doctor for evaluation. Good luck to you!

: I don't think anxiety disorder is the cause of acute heart failure (dyspnea and basilar lung crackles), especially there is change of early hypertensive heart disease by echocardiogram. You are quite young for primary hypertension with evidence of hypertensive heart disease so secondary HTN needs to be ruled out. You should be followed by a cardiologist, don't blame for just anxiety disorder.
