had a recent fall my ct results: normal noncontrast ct the brain. high left parietal scalp hematoma. its been 5days and i am okay in am but in the evening it starts to hurt and i get tired is this normal? or should i go get another ct?

: Trauma enough to create a scalp hematoma can give rise to migraine equivalent headaches a week or more after the event. By itself, hurting and getting tired are not reasons for a new study. I would confer with your doc who can decide if you need to come in for an exam. An exam could detect any abnormality that would require further imaging studies.

Puzzling question: A scalp hematoma means there is a collection of blood under the skin of the scalp. This usually reflects a broken blood vessel from trauma. It usually hurts, can persist for days or weeks, but has little if any importance. It is not by itself any sort of brain injury. It is not inside the skull.

Usually none: Most scalp hematomas will resolve without any surgery needed. The body will break the clot down as it does any bruise over time. Rarely if the hematoma is tense and expanding it can be drained by opening the skin and washing the area out. Those on blood thinners may have to stop them for a short time if there is an associated clot/bruise  on the brain.

I've never drained : One in 3 deacdes. They may take a few weeks to settle down & sometimes they can form a hard bony knot at the base of the collection.The body will eventually reabsorb the material & all will be fine. It is not worth the chance if infection or recurrent accumulation to drain these.

Yes, it can: Hematoma is a collection of blood in tissues.  This blood can track down under the force of gravity or pressure to the anatomic structures below, such as the face.  You want to make sure that the bruising on the face is not due to trauma to other structures. For example, raccoon eyes, bruising around the eyes, can be a sign of a basal skull fracture.  Please have this evaluated as soon as possible.

Yes: If small, may be able to accomplish under local anesthesia. If larger blood collection, best done in a formal procedure room with anesthesia.
