Maybe a fungus: You may have a type of fungus (ring worm) called tinea versicolor.  I would have this checked by your dermatologist.

Pale patch w/freckle: If there was a mole there previously, this could be something called a halo nevus, a mole that got inflamed for some reason and is being attacked by the immune system.  Another possibility is a patch of vitiligo (loss of pigmentation) where the pigment is coming back in one spot.  I would suggest an examination by a dermatologist for a proper diagnosis.

i have a mole on my sholder that is the same color as my skin(pale) it itches every once inawhile. is this bad i am really worried?

See your doctor: If a mole is changing or symptomatic (itch or bleed), you should see your doctor for evaluation and possible biopsy.

Discharge: it sounds as if it may be a yeast infection ask your pharmacist for a medicine to take care of it

Need more info: The skin of the scrotum needs to stretch and shrivel more than other skin so is never evenly colored, particularly if it gets any sun.  This may be a normal variation but without more details we cant tell. The most likely abnormal condition in a 19 year old male would be a fungal infection which can be treated with the same antifungals used for athletes foot. If you're sexually active get checked.

Could be anything: This could be a simple fungal infection, dermatitis or something serious such as a melanoma. See your doctor for a proper skin examination / dermoscopy to get a definite diagnosis. It is difficult to tell what it is without having a good look at the spots

Allergy: it could be an allergic reaction in spite of the black coloration the itching could be the best sign 
Take Benadryl (diphenhydramine) and see a doctor

Yes: 2 different issues blemishes are active lesoins that cause dark spots totally different treatments. Would coordinate with a dermatologist.
