hi, i have had several debilitating attacks of pain in the tip of my big toe, not in any joint.  i've been to a rheumatologist, podiatrist, had x-rays, blood work and needle extraction and i keep being told it's nothing. but the top of my toe gets swollen

If closed toe shoes worn, make sure they are well fitted and stuff  some Lamb's Wool into the toe box.

How old are you???: it depend on your age if you are middle age it could be gout arthritis.
or it could be regular arthritis.
or it could be bunion.
if you are smoker or diabetic could be a vascular pain.
see age matter.
see your doctor if pain persisted.

Granuloma Pyogrnicum: Sounds like you may not have the complete picture.  Exuberant granulation is caused by chronic foreign body tissue reaction whereby chemotaxis of inflammatory infiltrate and leukocyte cells macrophages etc cause angioneogenesis in an attempt to encircle an ingrowing nail or thorn or other material.  This in turns forms excess tissue growth which may in turn epithelialize when rejected foreign body.

Toe pain: Do you have anything else other than the pain? It is not clear from your post as to why silver nitrate was used. It's not used for pain. You do need an exam of your toe because without it, it's essentially impossible to arrive to a correct diagnosis.

R/O PLANTAR WART: Plantar warts are commonly found on feet and hands . The dark spot in the middle is the vascular supply. It can be treated with FREEZING/LASER/TOPICAL MEDICATION. Also it may be a corn or callus. Seek evaluation by a good podiatrist.

Yikes.: The tips of toes, and fingers for that matter, just don't like to be sliced. They are typically a painful heal and the injured pain receptors will take many months to calm down completely. The numb lump is either part of the initial injury or secondary due to an altered gait. Either way, it too, will more that likely pass. I speak from experience.
