Usually underlying all of these diagnoses of the foot are altered gait mechanics and the way that pressure is transmitted through the foot. Therefore evaluation by a podiatrist and consideration for custom orthotics and possibly even new shoes is appropriate

Not sure: What you mean by his has no skin. Does he have an ulcer? Unilateral calf swelling should be checked for possible clot. 
If there is indeed no skin an ulcer is present he needs to be evaluated by a doctor as well for proper wound care.

Possibly a sciatica: A sciatica is caused by an irritation of a nerve in the back that then can cause pain that runs down the leg like you are describing.  There are also potentially other causes as well.  Usually a detailed history and exam can determine the cause.  Thank you for your question.

Inflammation: If there has been no injury,then acute inflammation such as seen in tendonitis,arthritis or gout must be suspected.If you don't get prompt relief with NSAIDS like ibuprofen, or if there is associated redness or swelling, or severe calf tenderness, get examined by a physician for diagnosis and treatment. Appropriate tests will be ordered after the evaluation.

Pes Planus/flat feet: These symptoms are very similar to complaints i see with patients with hypermobile, or flat feet.  While walking, the lack of rigidity fails to absorb shock, and as the foot pronates, or "flattens" and moves outward "abducts", the calf muscle (gastrocneumius) is overstretched. Orthotics with a heel lift, for support, and stretching exercises can be very helpful.

Tendonitis likely: Sounds like achilles tendonitis if the pain originates from that thick tendon on your heel up to your calf.  The most dangerous thing would be a blood clot in your leg, typically occurs after travel or sedentary lifestyle and you get swelling in leg.  If calf is swollen and you recently traveled, see your doctor quickly.  If tender on the tendon anti inflammatories would be best if stomach OK.
