Since a guy usually cannot tell if his testicle is twisted, an exam by the doctor is needed.

Maybe the ER: If it is sudden severe testicular pain you have to go to the emergency room. It might be a testicular torsion, meaning the testicle is twisted and is not getting enough blood supply. This is an emergency because the testicle might die from this.  You would need quick treatment.
