Depends: One would  not know it  a cyst is a serous cystadenoma unless it was removed and d examined by path. It is  got too large or was under a lot of pressure it could rupture. When a cyst ruptures surgery may be needed if there is internal bleeding or severe pain.

Unfortunately Not: The problem with ovarian tumors is that you can have normal cell types adjacent to borderline or malignant cells. The ovary is notorious for hiding cancer cells which is why it's impossible to "eyeball" or sample an ovary and get a complete diagnosis.  That's why often the ovary is completely removed if malignancy is suspected. Cancer can also increase in stage if spillage occurs so it's not easy.

Yes: Naked eye can not distinguish cystadenoma with focus of cancer, ( has high incidence of cancer ) only after total removal and analysis by the pathologist , will establish diagnosis, & stage , a potential curable disease has to be treated with radical surgery with aim of cure.

Yes possible: It is possible longstanding cystadenoma some times can transform to malignant cystadenoma , pathologically it is cancer  or not a cancer , pathologist will never describe as borderline  cancer.
