Not cancer: A dermoid cyst is a benign type of turmor containing multiple types of tissue and has a characteristic appearance under sonography.  It will always have a complex appearance and the mural nodule is simply part of that.  Most dermoids gradually get bigger and have to be removed at some point, but if it remains stable it could be watched.  Surgery can be done day surgery with small incisions.

Not right now: Tell us why was her Ovary checked in the first place? Does she have any symptoms? This cyst can get bigger and the mural nodule can enlarge. This should be re- checked in 3 to 6 months time. if these show further growth then surgical intervention may be required. Find a good Gynecologist to help you sort this out.

Iodine: The ovaries require iodine and you may be deficient in it. Try to find someone who does an iodine loading test. If you are low, taking a safe iodine/potassium iodide supplement may just take care of your cysts for you.

It is suspicious: You should ask your doctor what it means in terms of future follow up. Since it is suspicious, a repeat imaging study needs to be done in 2 months. I would also advise you to find out if a ca125 test has been done or not? This might provide further guidance as to the possibility of ovarian cancer(if the ca125 is high now, removal of this lesion(cyst) should be considered sooner than later).

i just had an ultrasound that detected a 6 cm ovarian cyst with nodule, will this need to be removed?

Yes.: The fact that the cyst contains a solid nodule makes it more likely to be a tumor (either benign, or hopefully not-malignant), and make it important to have surgery to remove it. There are some lab tests that your doctor can order that can help to stratify it as higher or lower risk to be a cancer, but it should be removed.

Maybe maybe not: At age 30 it is more likely to be a benign tumor like a teratoma, but it sounds like the proper work up is being done

Your doctor will gui: The doctor who has ordered your tests suspects that you might have Cancer of the Ovary showing as a Cyst. But we need to have all the tests done, particularly CA125 before we can get any certain idea. A high 
 CA -125 level is common in most cases with cancer of the ovary. But it can sometimes be high for other benign conditions but the elevation is more modest in those other conditions.
