Ocular migraines: Ocular migraine can cause visual disturbance (usually on one side).  You should consult a neurologist right away.

Optic nerve: Your symptoms and MRI findings warrant collaboration between a neurologist and ophthalmologist. Entities like optic neuritis and increased intracranial pressure,as in pseudotumor cerebri are possibilities.

i have optic nerve swelling i haven't been to the doctor in over a year. i still have migraines, tinnitus, eye pain, some visual hallucinations. is this related to the condition? should i schedule a follow-up? i'm very busy with college at the moment?

: As long as you don’t worry about going blind may not need follow up. It would scare the heck out of me.

Accepted Answer

: You describe Chronic Daily Headaches, which could be secondary to migraines.  The best way to confirm increased intracranial pressure is via a spinal tap.  Ask your neurologist whether this should be done.  Your normal weight does NOT exclude possible Pseudotumor Cerebri.  Migraine can be controlled by the use of CGRP class drugs, and ask your neurologist about this approach.

recent diag. of ms by pa at neurologists office.  mri: 10-12 non-specific lesions occurring over 10 years; no obands, no lesions on spinal cord or optic nerve. 2nd opinion from wfu ms clinic did not concur with ms; complex migraine. how to proceed?

: Let's assume you have had a full and thorough evaluation and that migraine has been confirmed and MS excluded.  Now, find a neurologist who focuses on treating headaches, and have your next decisions routed through that facility.  Might return to WFU, or any local clinic which could provide this intervention.

: Need to distinguish between episodic and chronic  migraine patterns as different preventative meds may be indicated.  Worsening pain and tinnitus, when supine, might suggest intracranial hypertension, but may be just a variant response for a migraine headache.  See  a neurologist and get this handled fully.

Accepted Answer

: Yes, it would be unusual to have significantly elevated intracranial pressure in absence of papilloedema, and it is correct that a direct pressure measurement via LP is the most direct and objective test.  But maybe have an ophthalmologist double check eye grounds using a red free light to check maculopapillar bundle fibers, a subtle sign.  Chronic daily migraine  can be broken up with CGRP's.

i'm having random migraine headaches with facial twitching. ct normal. eye doc found optic nerve swelling and recommended mri. should i have the mri?

Accepted Answer

You bet you should!!: Your facial twitching could be facial myokymia, or hemifacial spasm, and needs imaging.  If you have optic neuritis, you likely have an inflammatory neurological disorder.  Multiple sclerosis may be answer, and if so, you could start effective and successful treatment which could bring this under control.  Hey, it's your choice!

Accepted Answer

Yes: It's possible to have a migraine without a headache.  This is referred to as an ophthalmic migraine or atypical migraine.  You should consult an ophthalmologist and have a field of vision study.  Following this a trip to a neurologist or your family doctor with a copy of the test should be done.

how to be sure if my headache is migraine? my symptoms were spasms of pain in head just above my eye with gas.

Migraines severe : Any headache which causes you to have to go to bed and is made worse by bright lights or loud sounds is and can be considered a "migraine". Migraines can be triggered by a variety of different issues including facial muscle spasm, clenching and teeth grinding.
