i was diagnosis  with pleural  effusion secondary to sle.  up to now it has just been painful and sob. but now i sound  hoarse w cough.  is this good or bad?

Not good: Obviously, progression of symptoms is not a good sign.  Without an examination, it is impossible to know what is causing this new symptom.  You need to see your doctor.

Lung nodules: There are abnormalities at the edge of the lung, or the surface of the lung on ct scan. Nodules may or may not be on the surface of the lung. Atelectasis = compressed lung and there may be false findings. Most nodules are found on the inner chest wall, the "parietal pleura". Ct scan should also be reviewed by your lung doc.

Determine cause: Pleural effusions, or fluid build-up around the lungs, is not normal.  Pleural effusions can arise from infection, heart failure, liver failure, malignancies or trauma.  The treatment will depend on the cause.  Your doctor will need to run some tests and may need to sample some of the fluid to characterize it.  If there is a lot of fluid and you are symptomatic, thoracentesis may be required.

The main symptom is varying degrees of shortness of breath.
