Use of fillers (like injectible silicone to accepted materials) have potential of damaging blood vessels and causing tissue loss & bear the potential of entering into the venous system around the nose which can embolize to the cavernous sinus in the cranium.  Can be life threatening.

Most patients who have a bad outcome knew the relationship wasn't right but went ahead with surgery.

Very customized: You really need to see a plastic surgeon who does rhinoplasty surgery (not all do). He/she will assess your airway/breathing and what you like/don't like about your nose. What you need depends very much on your skin, nose shape, ethnicity, and cosmetic desires. Small incisions may be required on columella, around nostrils, and within the nose. Suture +/- cartillage grafts under skin of nose.
