Very customized: You really need to see a plastic surgeon who does rhinoplasty surgery (not all do). He/she will assess your airway/breathing and what you like/don't like about your nose. What you need depends very much on your skin, nose shape, ethnicity, and cosmetic desires. Small incisions may be required on columella, around nostrils, and within the nose. Suture +/- cartillage grafts under skin of nose.
