: Was this a computer interpretation or the official interpretation after a staff cardiologist did an overread which is customary? I've been an EKG interpreter at 2 hospitals for 40 years and in my experience 20% of computer interpretations required alterations by us readers. Some of these altered interpretations were necessary because the computer couldn't compare the EKG with prior readings though

can troponin levels predict future heart attack? normal ekg, troponin levels very low chest xray good. i never know when to go when i have chest pain or pain between my breasts. been there several times, always got a clean bill of health. thanks?

: Current normal troponin levels are not predictive of future heart attacks. Tight follow up with expanding work up for causes other than heart or lungs for your chest pain in the outpatient setting may help in deciding when to go to ER based on pain's characteristics.

Normal,then elevated: Cardiac markers including ckmb and troponins measure damage to the heart muscle which occurs in a heart attack - the damaged/dying muscle will leak these into the serum which can be measured as blood tests.  They start off "negative" - in the reference range - and then 4-6 hrs after the start of the heart attack they begin to rise - will eventually peak and over several days  return to normal.

No: Stress and anxiety do not typically raise troponin levels. Medical conditions such as severe high blood pressure, congestive heart failure, renal failure are associated with elevated levels of troponin. Elevated troponin is not the cause of heart attack, but can result from one.

: Left are pain without any other symptoms in someone your age is very unlikely to be from a heart attack and more likely to be a musculoskeletal issue. What were you doing when this pain started? Does the pain increase with certain movements? If this pain continues, you should see your doctor. Good luck to you!

Very elevated: Troponin is quite sensitive for myocardial injury. If that super elevated troponin was due to heart damage I would expect very obvious ekg changes as well as clear cut symptoms making a diagnosis of an acute myocardial infarction very obvious. I wonder if the blood sample was damaged and that hemolysis of the blood sample rendered an extremely high amount of troponin. The attending MD knows best

: Ask their doctor how they interpreted the troponin. It is a marker for heart muscle damage but it's unlikely this was a heart attack at age 35. I suspect they have rhabdomyolysis which is caused by breakdown of skeletal muscle [from trauma or repetitive actions like playing congas] or dehydration releasing myoglobin into the blood which can damage the kidney. Troponin is high in Rhabdomyolysis too

Any heart problem: Troponin is an enzyme released by damaged heart muscle.  While the major cause is a heart attack, and large increases are usually due to this, almost any condition affecting the heart can cause small increases.  Viral infections of the heart, pulmonary embolus or medial procedures such as angioplasty often result in increases of troponin.
