my neurologist did a ncs and emg, and found: "severe ulnar entrapment at multiple levels".  could you, please, explain this diagnosis for me?

Ulnar nerve: The ulnar nerve is a nerve that helps control the lower arm.  It controls muscles that help clench the fist, flex the wrists and fingers.  It gives sensation to the inner portion of the forearm and the ring and pinky fingers.

Although usually injured at the inside of the elbow (the 'funny bone'), your nerve has more than one site of injury.  I would discuss this with your doctor to learn more.

negative emg - diagnosed with ulnar neuropathy. is surgery still beneficial as a preventative measure (to avoid muscle wasting) at this stage?

It depends : If the ulnar neuropathy was demonstrated from an imaging study showing a compression or other specific cause surgery might be beneficial but the lack of correlation to the EMG/NCV is bothersome. Discuss further with your doctor about the benefits of surgery and the current diagnosis before proceeding would be a suggestion.

is it possible to get a nerve conduction test & not an emg? i have very severe anxiety with needles & the only way i would be able to do an emg is if i was knocked out. my occupational therapist thinks i may have trapped nerves on ulnar side?

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and it is possible that you may not need EMG. It would be helpful to consult your primary care provider and if needed a rehabilitation medicine specialist. 
Have a healthy diet, exercise 30 minutes/day, drink plenty of water, have safe sex, no tobacco alcohol weed or street drugs. 
Get HPV vaccine.

No Diagnosis: Emg is a test to establish a diagnosis, and to clarify what nerve problems exist to explain weakness or pain.  When the results are unremarkable, it means that it did not identify the cause.  It does not mean your symptoms are not real.  A non-neurologic  explanation may be required.

YES: It can be a superficial nerve not tested or test is done too early in course od disease.

Nerve damage/disease: You have normal conduction in both motor and sensory nerves that were tested. Additionally the muscles tested during the EMG did not demonstrate abnormal activity that might be seen with inflammatory, immunologic, or degenerative neurologic disorders of muscle.  The tests do not exclude all neuromuscular disorders, but you did not state why they were ordered.

what does a normal emg/nerve conduction test tell you?

No pinched nerves: A normal test would tell you that the nerves tested are not pinched or slow.  The normal EMG (the needle part of the test) would  tell you that the muscle are not showing signs of damage (to either the muscle itself or the nerves).
