Irregular sac: An irregular sac with a lower than expected level of hCG suggests that there is a problem with the pregnancy.  As with most cases either it will resolve itself or end of as a miscarriage.  I suggest that you have a serum progesterone drawn as well because low progesterone in a otherwise normal pregnancy can result is miscarriage.

Possible ectopic: At a hCG level of 3188 a pregnancy should be demonstrable in the uterus by transvaginal ultrasound. if not there is concern for a pregnancy outside the uterus. A diagnostic laparoscopy can help establishing the diagnosis.

5 wks plus/pregnant: At five plus weeks, it is not always possible to see a fetal heart on ultrasound.  Another ultrasound in "2 wks." (if that is what "usg" means) is appropriate, but if you are having bleeding, it may be a miscarriage. Get a quantitative HCG (serum) from your gyn provider. This can give definite indication as to whether your pregnancy is still viable.

Early Pregnancy: With a pregnancy this early, it is hard to draw any concrete conclusions.  You should have HCG done and repeated in 48 hours, and a repeat u/s in the next 2 weeks.  Hang in there, but it may take some time to figure out what is going on.  Good Luck.

Endometrial bx: Any abnormal bldg should be investigated. A thorough history and physical exam, especially a pelvi exam should be done. I would also suggest an endometrial biopsy, and a vaginal ultrasound. Depending the above findings, an iud, birth control pills, laparoscopy, ablation of the lining of the uterus or a hysterectomy may be indicated. Discuss this with your dr.

Repeat Ultrasound: After IVF, one can see several different scenarios. It is best to wait and repeat the ultrasound. Do not feel anything is wrong till a repeat ultrasound is done. Early ultrasounds can be confusing. sometimes, twin pregnancies happen even with 1 blastocyst transfer as the blastocyst can divide. However,  growth discrepancy can happen and one may survive and the other may not. Good Luck!

I : I agree with the previous comment. Your daughter should be seen by a specialist for evaluation and treatment. There appears to be an impasse in the duct and the collection of an active infection.

Yes: The yolk sac typically appears at 5 1/2 weeks, and the embryo by 6 weeks, on transvaginal ultrasound, so 'empty' sac at 5 weeks can be normal.

It may be normal.: Your dates may be off, or the machine used to scan your pelvis may not be good enough to see a yolk sac and an embryonic pole. For more details, get a transvaginal ultrasound and repeat in a week. Take Folic Acid 1-5 mg daily and best of luck!
