It's reproduced on palpation, and the pain can be aching and deep, and range from slight to lightning-like.  It may be caused by connective tissue diseases, poor posture, and emotional disturbances.  Treatment can vary from relaxation/physical therapy to antidepressants.

Migraine is a sudden onset of headache with or without neurological symptoms and nausea/vomiting.

Learning to manage: Some things get better, some things can be cured and a lot of medical issues we manage.  If a complete rheumatologic, infectious and metabolic workup was negative ie: lyme, vitamin d level, thyroid etc then you have to check lifestyle issues: obesity, smoking, exercise routines and diet as well as sleep patterns.  You may then need to see a well versed pain management specialist or physiatrist.

Injection: Consider an ultrasound-guided injection of either a steroid or a regenerative biologic injection such as platelet-rich plasma or an amniotic membrane allograft that could be diagnostic and therapeutic. Challenging: Would consider that you have a more widespread immune disorder, and the "lesions on the brain" may well be quite important.  However, you could also be suffering from a disorder such as cadasil or melas, and, based on your info, could consider a vasculitic disorder.  Since there are many potential answers, genetic testing, spinal fluid analysis, and additional docs might help.

Mouthguard: When you grind your teeth, you are over working your muscles and they are fatiguing. I would initially start with a brux guard or mouthguard to help reduce the inflammation of the muscles.  Talk to your dentist about a brux guard.

A long time can occu: techniques to help myofascial pain includes rest, and exercise. Local massage may also be helpful. This is a long-term program it will come and go and the better you can stay in condition the better your myofascial pain will do.

Too convoluted: Basically no. But. Depending on if you have a nerve damage sensitisation going on in the head and neck areas, it can make you more at risk for other neurogenic pain from procedures in that region. That would not be considered "myofascial" (soft-tissue), but more nerve involved.
