Osteorarthritis: Without knowledge about your age and history this is difficult to answer. You may have osteoarthritis (wear and tear type) and less cartilage between the joint.  Certain movements will cause intense pain since the cartilage and lubrication is less.  You need to be seen, xray may help confirm the dx, however MRI may be needed to look for soft tissue and ligament.

Chondromalacia: There are several possibilities that could explain what is going on.These include patellar tendonitis, juvenile rheumatoid arthritis, patellofemoral syndrome, patellar chondromalacia,hamstring spasm amongst others. It is best to see a Sport medicine or PMR specialist for a thorough evaluation and referral to PT for quad training.In the meantime ice and anti-inflammatories as needed are recommended

INFLAMMATION: This sounds like mild inflammation of the supporting muscles and tendons of the patella ( kneecap). Many causes are possible:if you are exercising- remember to warm up and cool down.  Certainly simple passive flexion and extension of the knee will strengthen the quadriceps and supporting ligaments. I suggest massage with biofreeze. If not improving-orthopedics.

i have pain on my right knee cap when i put direct pressure on it ( when i kneel on the ground) it doesn't hurt 2 run or lift weights.what could it be?

Patellofemoral dys.: What you are experiencing is probably due to either prepatellar bursitis (some swelling on the outside of the patella) or inflammation of the back of the patella where is contacts the groove in the femur. A physician can help sort this out for you.

soccer player w/ knee pain, 8/10. no major injury, mri +xrays show nothing. worse pain in outside right knee. tried ice, rest, stretches, running. help?

You need to be eval-: Uated with a detailed hx and examination with review of the xrays, mri, and result of the physio etc.
Other causes, are most likely, be referred pain from the hip area or the back. These areas need to be checked out.
Also xrays ; mri's are known to not show all lesions of the cartilage, specifically the articular cartilage.

left ankle sprain now my right knee is in pain when i sit and walk. been alternating cane to keep pressure off ankle and knee. is the knee pain from overusing it? what assistive device do you recommend.?

Yes: Yes the knee pain maybe due to compensatory changes in your walking. Try crutches . Can get a pair at a medical supply store.

You need an evaluate: This could be an internal knee problem you need an appropriate evaluation . Is it Muscle , is it a tendon is a bursa, is it joint, or is it Cartlidge. My biggest concern from what you tell me is it could be one of these  - Cartledge would be the one I would be first thinking about

Something is wrong: but cannot say in your particular case, but generally, "barely carries my weight" does NOT improve with continually walking. Stiffness does and that is a function of the inflammatory fluid in the knee. Besides aleve (naproxen) and an acewrap, there's lots orthopedics can do.

Pain behind knee: When the focal area of knee pain is in the back (called the "poplieteal fossa"), one disorder to suspect is a large cyst behind the knee joint, which is known as a baker's cyst. An ultrasound test can be used to confirm this. Treatment is either wait and watch or surgical removal of the cyst.
