You'll need to check with your brother's doctor or someone else in your city or village for advice about which antibiotic(s) are likely to be effective. The lower pole: Is the lower portion of the affected kidney.  An isolated cyst, if small and no renal compromise is identified is typically just an incidental finding during a scan (ct or ultrasound) for something else and just needs to be periodically observed, but not treated.

Nephrologist: Small sized kidneys  means kidneys appear smaller on ultrasound images. This must be evaluated in relation to the body size, age and renal function. On kidney ultrasound, kidney appearance is usually compared to liver , if kidneys appears more enhanced , it usually represents parenchymal disease which may be due to many causes like diabetes, hypertension etc. You must discuss with nephrologist.

Cystic Disease: Typically small sized kidneys can be congenital(born like that), or in many instances as your kidney function gets worse the kidney scars and get smaller- there are exceptions to this as diabetes kidney disease, HIV related kidney disease, infiltrative diseases(lymphoma) etc can be large. if you have small kidneys as well as cysts- this could be related to underlying disease- complicated answer.

Kidney pain: Its really to say what's causing your pain with having the opportunity to performed a physical exam. Kidney pain is usually caused by distension of the renal capsule, as in stones, infection, and masses causing obstruction are the most common reasons. Trauma of the kidney can also cause pain. Enlarged renal cysts may also refer pain to the back. A exam +/- MRI would help clear up the diagnosis.

i'm in severe pain from 9.1cm cyst on kidney and told not to exercise. is there a reason i can't just get it to burst? it's simple and on my nerves.

Here are some...: Almost always, a large simple cyst of kidney alone will not burst or cause pain. To deduce if the pain is related with this cyst, a detailed Hx &amp; proper physical exam before timely imaging study are needed. The only way to prove if the cyst is cyst-related is see how your pain may evolve and resolve after cyst-related surgery. So, ask doc timely as needed.
