Muscles: Usually this is caused by cramping of small, [intrinsic] foot muscles that are in the foot that help stabilize the toes.

Insurance  coverage: Usually insurance will cover the procedure unless it is solely for cosmetic reasons. It can be done at a hospital or surgericenter as an outpatieint.

toes/top of foot swollen. i can't seperate my toes. icing causes more pain/swelling. feels like it weighs 10 lbs. swells all the time. hurts every day for the past 3yrs. why is this happening to me?

Needs Full Work-up: This could be simple osteoarthritis, especially if you had an injury. Or rheumatoid arthritis. Or a nonunion fracture. Or poor circulation. Even renal failure.
Please have a evaluation with x-rays and exam.

i smashed my foot pretty hard against a sofa and my fourth toe (beside my pinky) crooked to the right. i turned it back in place but it looks off

Could be fractured: The toe could be fractured to a more or lesser degree. It will eventually heal on its own, but may stay somewhat crooked. If you think that may bother you, see a  surgeon. The can do anything between simply giving you a cast to straighten the toe out and doing reconstructive surgery.

Syndactyly: The risks are the same as with any surgery.   However if this is not causing problems why have the surgery ?

What : What you're describing is called syndactyly, or "webbed" toes.  You would think that all it takes to separate the toes would be a simple snip-snip and sew up the openings, but this is not the case at all.  There is not enough skin to close everything with just a nice snip between the toes, so the incisions are longer and fancy to create skin flaps that are used to cover the open areas.  It's not so simple, and should only be performed by a surgeon experienced in such things, and since this surgery is sorta rare, there aren't too many people around that do a lot of these procedures.  In fact, in my neck of the woods, it's typically done by a podiatrist with a plastic surgeon.  Plastic surgeons are the best at creating skin flaps and great-looking scars (the last thing you want is to exchange your webbed toes with a thick scar between them that is a constant source of irritation).

The other concern is that sometimes (this usually happens with accessory (extra) digits more than syndactyly), the nerve and vascular supply to one digit is much more significant than it is to the other digit, and separating them runs the risk of cutting off blood supply (and feeling) to the digit with the lesser neurovascular attachments.

So.... Given what you now know, are you sure you want your toes separated?

: The weight could put strain on the skeletal structures, so weight management may be helpful in reducing the strain that is causing pain however what is more important may be the foot wear. You may want to suggest that your friend get a good foot wears that provide support for his feet such that there is less strain especially with the type of terrain he may have to walk on.

Custom orthotics: Are often helpful in controlliing biomechanics of lower extremities. Problem could be originating higher up in knee, hip or back which would also need to be addressed.

i have a piece of skin that has separated under my foot. it's seems to have grow detached like something is in it. ?

Tag, your it.: Anything growing, partially attached, from the bottom of the foot needs to be examined. A specialist should be able to quickly determine the proper treatment plan. My concern is if this piece continues to grow and/or gets ripped loose, an infection could be started and things could get a whole lot worse. It is best to have it examined and remove all doubt.
