Insurance coverage: Usually insurance will cover the procedure unless it is solely for cosmetic reasons. It can be done at a hospital or surgericenter as an outpatieint.

Varies: It depends on if the doctor is in your insurance network or not. Check this out by calling around. Does the webbing cause you any pain? In and of itself, that condition shouldn't limit your abilities to walk, run, swim, etc. Why do you want to go through a surgery?

It depends: This is not a simple answer. Some people actually have only one artery and nerve in the webbing and separation is not a good idea. Sometimes it's just skin so it's relatively straight forward. I would go for a consult with a board certified foot and ankle surgeon.

Several thousand $: A desyndactylization or separating your toes could jeopardize the circulation to one or more of your toes and you could lose them. I would leave them be and be a good swimmer.
