im suspected ms due to physical symptoms and lesions found on my brain. my dr found inflammation behind my eye i have no symptoms of optic neuritis. could this inflammation be ms itself causing swelling or is optic neuritis possible with no symptoms?

: There is a correlation of the occurrence of optic neuritis and MS.  See an ophthalmology physician and consult a neurologist for guidance on MS.

: The pain of acute optic neuritis can be variable, more often noted on eye motion.  Proximal inflammation of optic nerve may be relatively pain free.  Optic neuritis does not always mean MS, as a number of other  entities must be considered, but depending  on prior studies, about 60% of cases wind up "clinically definite MS".

Your doc is right.: Ms is never diagnosed the first time you have symptoms.  It is likely, but you can have other reasons for these findings.  Just stay aware of the symptoms and talk to your neurologist about them.

: Besides multiple sclerosis, optic nerve inflammation (optic neuritis) can occur with other conditions, including infections or immune diseases, such as lupus. Rarely, another disease called neuromyelitis optica causes inflammation of the optic nerve and spinal cord.

Doubt MS: Your symptoms suggest possible migraine, and, your grandma's diagnosis provides minimal statistical risk.  I am somewhat baffled as to why this diagnosis is being considered based on information provided.  There are many fine neurologists near your home, and suggest a consult to assist in future evaluation and management.

taking copaxone (glatiramer) for many years for my ms . i have pain in my right big toe with inflamation .  could my body systems not want me to take it .... gout?

Need to reassess: At your current age, 77, you may no longer need copaxone, (glatiramer) and if you developed a secondary progressive ms transition, this drug would not be appropriate to handle that either.  The pain in your toe could be gout, but other explanations may be considered.  Yes, check your uric acid, but also update MRI of brain, and get full neurological re-evaluation.

No: Dilated perivascular space is not a typical finding on MRI for ms. Subcortical "white" matter plaques are the classic findings. Moreover, the lack of change over time is reassuring. Sometime a lumbar puncture with appropriate testing can be helpful in making the diagnosis.

Multiple : Multiple sclerosis is a complex syndrome caused by imbalance in the body's own immune system that leads it to attack something called myelin, a substance that insulates nerve cells in the brain and spinal cord.  These attacks leave temporary and permanent scars on the brain and spinal cord that can be seen on mri.  Symptoms include loss of vision, or loss of feeling or strength in a limb, that may appear and disappear, only to show up in a different spot later on.  Spasticity, a condition resulting from poor control of muscle contraction and relaxation, can cause muscle pain, and usually occurs later in the course of the condition.

Multiple sclerosis is often thought of as a condition that mostly affects young, white women, but this is a stereotype; men, non-whites, and older people are also affected.

Diagnosis of multiple sclerosis is done according to an international standard known as the (revised) mcdonald criteria.  These criteria are based on clinical findings, MRI findings, lumbar puncture (spinal tap) findings, and special testing (like evoked potentials).  There are also a number of other conditions that can resemble multiple sclerosis symptoms, MRI appearance, and laboratory findings, that may require additional testing to exclude.

The less "typical" the presentation, the more important it is to confirm the diagnosis.  It is never wrong to consider getting a second opinion.

It can: Everyone undergoes stress on a day-to-day basis. The  Fact is that is the natural condition for any living organism on this planet. However, undue or unnecessary amounts of stress causing psychological harm can change the function of the immune system. This theoretically, could affect people with multiple sclerosis since it's activity is mediated by the immune system.
