Not really: Tongue tie needs to be surgically clipped. If you try to stretch it out, i guess theoretically you could tear it. But this would cause significant pain and probably bleeding. I would not recommend it.

Doubt: Not that i know of. I think he probably is just endowed with genetically full lips. Some people  'enhance' their lips with various fillers. Look at lisa rinna for example.

Speech therapist: You should be evaluated by a speech therapist. I'm sure there are other words that you have difficulty with.. Have yourself evaluated and treated. You'll be enunciating perfectly in no time!

No: Being tongue tied is not a painful condition. Would think more on the line of gum disease/gingivitis or canker type sores in his mouth. This time of year hand/foot/mouth disease is also a consideration, but he is a little bit on the old side for that (we usually see that in the 3 and under age group, but not impossible for a 4 y/o to have it). Do good mouth hygeine, and see doc if doesn't resolve.
