Pulmonary TB: This probably that you may it's possible that you may have active TB, or inactive TB, or scarring from prior lung infection.  
if you are having symptoms of active TB like coughing/weight loss/fever/night sweat/coughing up blood.  you should see a doctor immediately to get treatment.  if you don't have symptoms, you should still see a doctor to see what should be done next.

Accutane is a good d: Accutane is a good drug for the treatment of acne and quite effective but you will need to be checked and guided by a skin specialist(dermatologist). It has side effects also, so it has to be carefully supervised by a physician.

: It sounds like there wasn’t a full deep breath taken for this x-ray. So, at the bottom of the left lung there is a minimal white line on the x-Ray. This white line could be a non-expanded piece of lung (atelectasis) or a scar or just the technique of the film. If there is concern, a CAT scan of the chest could be obtained and a lung doctor seen. was given surfactant as an infant due to extreme prematurity, now recent lung ct shows inflammation, minimal bilateral scar and unspecific aveolar density. i also had covid recently. could this be due to the surfactant or covid and can it be reversed?

: Surfactant is a product that mimics a natural one made in the lungs as it matures. I doubt it has anything to do with your issues 30 years later. What is common for those infants that needed surfactant to survive is residual lung scarring and sensitivity to respiratory illness for their lifetime. No such entity: If your pain is getting worse, then you need to have a medical examination done either by your PCP(Family Physician) or by your Gynecologist. it is hard to guess the cause of a pain like this unless it is on the scar itself which does remain sensitive for 1 to 2 years after surgery(I assume that you have had a recent appendectomy done...?date)

Possibly: Eczema is frequently well controlled with cortisone type topical medicines. If there is full thickness scarring then improvement will be limited to the extent of damage. The damage most often comes from the excoriation caused by scratching to relieve the itch often accompanying eczema.  Antipruritics therefore are important in treatment.
