FNA: The size and features warrant a FNA. Go for it and see what it shows. Risk of cancer in thyroid nodule is low, slightly higher with microcalcifications but still low.

Should be biopsied: thyroid nodules are extremely common, and 80% of nodules like yours are not cancer. However, once a nodule is over 1 cm, the typical recommendation is to biopsy the nodule using a fine needle. This is done in the office. You can have it done by an ear nose and throat doctor or a surgeon. Some surgeons specialize in thyroids too.

Not enough info: I'm not sure of the question.  Is this from a breast ultrasound?  Specific ultrasound features of the nodule, combined with mammographic and physical exam findings,  will help determine whether this nodule should be biopsied, or followed up at 6 month intervals.

Nodule: Hypoechoic generally means that the lesion is more likely to be cystic (contains fluid), however, increased vascularity is concerning. depending on the history, location and presence of other symptoms fine needle aspiration for cytological analysis or a biopsy should be considered. Watch for alternative points of view from other physicians

Probable Goiter: I assume this is the thyroid gland. Often the the gland enlarges and contains numerous nodules representing various entities. This is referred to as a "Multinodular Goiter" and is usually benign. There may be functional or hormonal abnormalites which can be measured with blood tests. However, each nodule must be inspected individually to ensure it is not cancer. Biopsy may be the only way to tell.

: Not always. Increased vascularity in a thyroid nodule doesn't always predict malignancy. I'm a firm believer in having the endocrinologist and an experienced head and neck surgeon collaborate in these cases in providing best advice and follow-up.

Indeterminate: Overall risk of malignancy in a solitary solid thyroid nodule is up to 25%. Ultrasound guided needle biopsy of such nodules is therefore frequently recommended.

Needs FNA: Most nodules of the thyroid are benign.  However, fine needle aspiration (FNA) biopsy of a nodule is indicated in your age group for nodules over 1 cm in size.  Having increased vascularity raises the index of suspicion slightly.  You should have thyroid function (free T4, TSH) checked prior to biopsy.
