Fever activates the immune system and makes it harder for the infection to survive. My advice generally is to treat the child, not the fever. If he feels bad, treat. If not, don't. See a doctor if the fever persists.

Has to drink: Kids who don't feel well don't eat well.  They make up for it the day they feel better, so don't worry about that.  The drinking is more significant.  A child who has fever, doesn't drink, and is vomiting is at risk for dehydration.  Give small sips of liquid, maybe as small as a teaspoon, every few minutes.  Even if he vomits, the body will absorb some.  If it continues, see your doctor.

Not likely,: Seasonal flu is very common viral infection. Usually the condition resolves on its own with home care. Drink a lots of water and good rest. NO ASPIRIN!. If concerned, see your pediatrician.

Still probably viral: At four days it is still likely viral.  Would treat symptoms for now.  Irritability can be seen with ear infections especially when laying down.  Consider seeing md if there is no improvement after 3-4 days or if symptoms worsen.

A treatable illness: Fever is a natural body response to illness that increases heart rate to push the bodies white cells and antibodies around faster to fight illness. By itself it can be ignored and doesn't require antibiotics. If a kid has an identified bacterial illness that would benefit from an antibiotic it would be justified.

Pharyngotonsillitis: Physician may have felt confident that he/she is seeing strep throat without testing.  Hard to second guess without examining child.  However untreated strep throat can result in rheumatic fever. Contact doctor before deciding not to take antibiotics.

Few possibilities.: How long do you have fever, have any h/o ear infect., sinusitis, bad teeth . Feeling cold means the chills you have , i assume, from release of infective toxins into blood  causing  fever also.Any fever can cause head ache, if high.In your case it is low. Tonsils infection also to be ruled out too. But early meningitis, tuberculoma brain-need to be ruled out. Pl. Visit your doctor, need more history.

TB &amp; cancer: Tuberculosis can affect organs like kidneys,spine etc
Some cancers evade all testing.
Keep account of all your symptoms with dates.
Was urine sent for TB culture? Did you consult an infectious disease doctor?

Rehydrate as needed: Besides taking over the counter pills, I assume.  Depending on how long you have had the cough and mild fever you may need either an antibiotic or continued use of over the counter medications.  Normally a flu like illness will peak at day 3-4 and then start getting better. Cough may last 6 weeks but fevers should resolve. Otherwise you may have a bacterial illness that requires and antibiotic.
