Endplate sclerosis  is bony thickening seen at vertebral disc area.

Many options: If your doc has thoroughly evaluated you and is confident your pain is facet joint-related, there are several options. Oral antiinflammatories or steroids can work for acute pain. See Below: The L5-S1 facet joint is an articulation between the L5 vertebral body and the sacrum.  You have one on both sides and this is normal.   A pseudo-articulation is not a true articulation which may be suggesting a transitional vertebra. This means your L5 vertebra may have a large transverse process touching the sacrum and creating  a pseudoarticulation or "sclerosis".  Google Bertolotti Syndrome

The facets are: small joints between the posterior parts of adjacent vertebra.  Sclerosis generally implies degenerative arthritic changes.  Lower two levels refers to the facets involving the lowermost 4 vertebra(each facet joint involves 2 adjacent vertebra).

Connecting bones: Facets are the connecting bones of the vertebrae and each vertebrae has inferior and superior facets connecting to the lower and upper vertebrae. Sclerosis seen on imaging usually means hypertrophy of enlarged facets which often indicates a chronic overuse, past trauma or injury, and/or arthritis. It may be causing your back pain but if causes any radiating symptoms you should speak with your doc.

Either: Neuro/ortho spine surgeons can both treat facet arthritis surgically. However, great caution should be taken, as methods to reliably identify patients who will do well with surgery for facet arthritis has not been established by scientific evidence. Outcomes for facet pain surgery are difficult to predict. Some patients are made better, while others can end up worse.

The MRI shows that level has become "worn down" leading to degeneration of the disc and the facet joints, sclerosis, and a slight slippage (retro listless) of one vertebrae on the other.  Don't hesitate to be seen.  Many good treatment options available.

Clinical exam: Nerve damage causes specific symptoms, and shows on a clinical examination. See your md for an exam, if in doubt they will send you to a neurologist.  Should suspicion of nerve damage be found, electrophysiological tests may be used to confirm. The minor changes seen on xrays and MRI you describe would not cause any nerve damage. If it's a chronic pain situation - read john sarno's book.
