If you mean breast..: Implant capsule contractures, the answer is murky because we don't know their cause. Only a small proportion need treatment: the ones that are painful or cause distortion of the breast that can not be hidden in clothes. Sometimes, if the scar tissue is removed and the implant replaced, the scar tissue will not recur, but this is unpredictable. For a very small number of patients, this is a problem.

?: What exactly is "capsular contracture hardening"? Contractures are often "released" by opening them...They generally do not "harden" there can be bone under the capsule that "spurs" or has risdges and this can be smoothed. Hope that helps.

how common is capsular contracture after breast reconstruction? a friend had capsular contracture after her augmentation and said it was very painful. i'm worried the same thing could happen to me after my breast reconstruction

Capsular : Hello, 
capsular contracture occurs after breast augmentation at a rate of less than 10%, usually in the range of 5 to 10%. The outcome after the operation during the first year is generally predictive of whether someone will develop a bad capsule. 
Measures to moderate capsular contracture after breast augmentation include range of motion of the shoulders and gentle massage of the augmented breasts.

The severity of capsular contracture varies. Mild capsules are generally not very symptomatic and not very bothersome, ranging up to very severe rigid fixed firm capsules that are uncomfortable. Fortunately, this is rare. Work with your plastic surgeon after augmentation in pursuit of the best, softest, most natural result you can obtain.

Thei : Thei first goal is to prevent capsular contracture, this is done by making sure the breast pockets are free of blood and infection. Placing the expanders and subsequent implants under the muscle helps to reduce capsular contracture. Early evaluation suggest that the use of accular dermal matrix may help reduce capsular contracture. Appropriate drainage will help divert blood and other fluids that can lead to inflammation.
I personally believe that early massage with a castor oil based serum is very helpful in maintaining soft pockets. This massage should be done for 4-6 weeks after each surgery.
Once a capsular contracture occurs, it may successfully be managed early with massage, castor oil based serum, and the use of an asthma medicine, such as singular or accolate (zafirlukast). Surgery may be required in those patients who do not respond to non-invasive measures.

i am planning on getting breast augmentation (b to dd). my surgeon (at a very well respected hospital) is recommending implants above the muscle. i thought it was better below the muscle r/t decreased risk of capsular contracture. thoughts on this?

Breast implants ?: It may seem tempting to aspire to a "DD" from a "B", but any acquired benefit from this kind of change is balanced by problems associated with the size attained.  Best results are with implants beneath the pectoral muscles. Get a second opinion.

: Consult an orthopedic hand surgeon specialist, as such contractures may eventually interfere with some activity or job requirements. "CP" at birth may not be a reversible entity, however.

Hand and fingers: If you have a dupuytrens diathesis not just a contracture it may also affect the sole of the foot and toes, and the penis. Surgery indicated for mp joint contracture &gt; 20 degrees, or any involvement of the pip joint.
