Typically: you cannot bear weight on your foot.  It hurts in the mid foot or the long flat part in the top of your foot and will be swollen or bruised.  The best way to tell is to get an X-ray.

Rest and patience: A metatarsal fracture will take 8-12 weeks to heal like most bones in the body. Initially, reduced weight-bearing, ice and elevation for the first couple weeks helps reduce pain and start the healing process.  Progress of healing should be monitored by your orthopedic surgeon with x-rays.  Some require surgery if healing doesn't progress as expected.  Increase calcium intake to 2 gm/daily x 2 mon.

Because it takes: Approximately 6?Weeks give or take two weeks in either direction it is not unusual to have pain at the 4 week mark. I hope you are in a surgical shoe or walking boot and not walking on your foot without being in one of these.

Time in air cast: You must stay in the cast as long as your doctor instructs you to.  Patient's have variable healing times and needed to be followed with x-ray.  It will only benefit you to stay in the cast until told to remove it.

Hard to say: It is unlikely things could have changed so quickly.  It is probably more related to the techniques in which the serial X-rays were taken.  Sometimes variations in the angle at which an X-ray was shot or in the degree of penetration if the film can make the findings different.  But if you have little ir no pain and no fracture was seen on your last X-ray, I would not worry about it.
